Admissions Forms

Below you will find links to Langley Montessori's registration packets. There are two different sections, one for returning families and one for new families.

For your convenience each form packet is available in two formats. If you have MS Word then the Word version files can be completed on your computer. As an alternative we also have each packet available in a PDF version that can be printed and completed manually.

Please note that in order for us to confirm your child's registration all pages must be completed, signed and returned to the main office.
